Reactivation Recare Track Apppointments

Reactivation patients list: We have a patient list of last visit date up to 12/31/15 approximately 1400 patients. Our front team scheduled some of these patients and came in for the appointment i.e. 1/13/2016.
1. How can we track all the reactivated patients from the original last visit date to the date the patient came in? In another words we want to see how effective the reactivation was from the time of the call to the time the patient came in.
2. Including was diagnosed, production collections and did the patient schedule a future hygiene and doctor appointment and what date are those future appointments?
3. What report can we run to get these stats?
