Remove continuing care from no-show appointments.

When you enable the automatic continuing care feature (which is great) for newly scheduled patterns, it puts the current date in the "continuing care" field. When they complete the appointment, that field is updated to reflect the appropriate due-date. However, if the patient no-shows the field remains populated with the date on which you made the initial appointment.
