FQHC place of service default should be 50 rather than 11

FQHC's claims require a 50 place of service on claims in order to get the PPS rate.

    Place of service 50 for FQHC's is being dictated by CMS and should be built into this EHR System. We should not have to go and edit every claim we send to Place of Service 50.
