Add Priority Level to Patient Referred to Dialog

Allows FQHC's to prioritize referrals to know if the referral is emergent, urgent or routine, where staff would then work on the most emergent referrals first, then urgent requests and finally routine requests.

Patients and staff would both benefit from this, getting the most important cases in to be seen/sent out first.

Drop down field in Referred To dialog box, where a user could select Emergent, Urgent, or Routine. This would also be setup in Definitions, so additional levels could be added.
