Add effective and termination dates to fee schedules

This helps so the old fees don't populate on a certain date of service or vice versa. Some insurance companies only allow a certain fee for that year, otherwise, they will deny for new fee.

Add a feature on procedure code set up to mark if the procedure does not apply to the annual maximum. Example: DentaQuest does not apply removable prosthetics to the Medicaid plan annual maximum.This would allow for more accurate estimation.
