Add Clinic Filter to Referred by Doctor/Other Report

Request to add the option to filter this report by Clinic like it allows for other DXOne reports. Ideally, ALL DXOne reports should have the ability to filter by Clinic since this is an Enterprise system.

Have spot for personal pronouns that is easily visible

Personal pronouns can make a patient feel more welcome in the clinic and if it is easily visible on the patient chart/on the more information tab no one would make a mistake.

Need to be able to sort adjustment codes alphabetically

Since we cannot inactivate codes if we could at least have them sort alphabetically instead of in system addtion order it would make it easier for staff to pick the appropriate adjustment code
