add to the daily appt list in vers 5.0: option to select multiple providers/add field for "other phone #" and add field for email address

Please add the above options to the vers 5.0 daily appt list. Helpful to have in case of inclement weather/ we can print this list out w/that info to contact patients in case office is closed!

modify clinical notes reports

If we want to track, by provider, which clinical notes are not approved, we just need the patient name, date of procedure and provider name, not the list of all the notes. It's a waste of space on the report

Emailed reports should also be titled with the job name

Scheduled reports that are sent by email should contain the report type (ie. utilization) as well as job name in order to differentiate various reports that get sent at the same time.
